MODEL RELEASE/MEDIA WAIVER/DISCLAIMER

Salisbury & Associates, LLC dba Wylde Style Productions,CageZilla Fighting Championship and Thai Championship Boxing (“Promoters”) are dedicated to the promotion of Mixed Martial Arts and Mixed Martial Artists.  As such, we understand the need for media members of all forms to help us in the promotion of our events, Mixed Martial Arts and Mixed Martial Artists.  However, we must protect our rights along with the rights of our fighters.  Therefore, we reserve all rights to photographs, videos or other media types now existing or subsequently developed and to only allow photographs, videos or other media types now existing or subsequently developed to be taken and used for personal use unless the following waiver is signed by any media member intending to use any images taken for any use other than personal use.

By signing below, you affirm that you are at least 18 years of age or have a parent or guardians signature if under 18 years of age and agree to the following:  any photographs, digital depictions, video, audio or other content taken/shot will become the property of Promoters and/or any affiliates for any purpose, including, but not limited to: reproduction, disclosure, transmission, publication, broadcast and posting in any media including but not limited to: television, radio, print, the internet, or any other media now existing or subsequently developed.  Furthermore, Promoters will own and is free to use any photographs, video and audio, and any ideas, concepts or techniques contained in any communications taken/shot by Promoters for any purpose whatsoever, including but not limited to: developing, manufacturing, and marketing products using such information and developing stories or other creative content.  In addition, your signature below is your consent to release Promoters and its affiliates from any and all liability from any photographs, digital depictions, video, audio, or any other media now existing or subsequently developed that are submitted.  You also agree that you have read, understand and agree to the terms and conditions in Contest Rules, if any.
